Dear Oconto Unified School District Families,
My name is Stephanie Stauss and I am your School Nurse. I serve all schools in the district. My role as your School Nurse is to make sure that your child's health needs are meet so he/she can focus their attention and energy on learning. To be able to do this, I will need your help. Please review the following to be sure the district is appropriately meeting your child's medical needs.
If you would like your child to receive “as needed” medications next school year (Tums, Tylenol, Aspirin, Allergy relief, Cough drops…), please fill out the "Over-The-Counter Medication Permission Form" in the forms section. We cannot accept hand-written notes or emails as permission. The school has limited supplies, so it is your responsibility to send the medication to school with the student in the ORIGINAL packaging with your child’s name written on it. Please keep in mind we do not supply chewable or liquid forms of medication.
If your child has a chronic disease or medical diagnosis that requires prescription medication to be administered at school, the Medical Provider Authorization Form must be filled out by both parent/guardian and a medical provider. This form may be obtained from the office or is also located in the Forms section. The prescription medication needs to be sent to school in the original bottle with the current prescription label on the bottle.
Please check with your child’s medical provider over the summer to be sure your child is current with immunizations. You can check the immunization schedule and see Wisconsin's immunization requirements in the "forms" section of the website. If your child does not have the appropriate immunizations, he/she may be excluded from school and this could result in a fine up to $25/day. If you wish to sign a waiver for your child’s immunizations for medical, personal or religious reasons, please check the waiver box and provide a signature on the bottom of the Student Immunization Record. This form is also available in the Forms section.
It is very important that the school be informed of any medical needs your child may have (allergies, asthma, seizures, diabetes, chronic health concerns, ect.). Please fill out the Health Information Form that was sent home with your student at the beginning of the school year (also available in the Forms section). If your child has any medical diagnosis, I strongly encourage you to set up a meeting with me to discuss special instructions, emergency plans, and accommodations needed within the school setting.
